Health insurance in Japan, how does it work?

question for the locals...

I was wondering how the medical insurance scheme works in Japan.

Is it like here in the UK, where it's provided by the state, aka "free" ?

or is it like in the States, where everyone has to have paid medical insurance?

or is it like a combination of the two, where there is a state run scheme, where you can if you can afford to, also take out a private insurance policy?

There's a National health insurance that's based on your income. If I didn't make that much, I don't pay too much. You can also have health insurance issued by your company. My company pays for mine, & handles all the paperwork each month.

In addition there is some supplemental private insurance for specific things like cancer but its a very small part of the market.

Actually they have been in Japan for a long time (since the 70's) and this is AFLAC's most lucrative market I believe.

Aflac is the number one life insurance company in Japan in terms of individual policies in force and the largest foreign insurer in Japan in terms of premium income. Aflac also ranks first in the number of individual policies in force among all of Japan's life insurers and is the fifth most profitable foreign company in any industry in Japan.

On July 2, 2001, the Aflac Duck went to Japan-the first time in Aflac's history that the company has taken a U.S. ad campaign to Japan. The campaign, which was rated as one of the "ten most effective campaigns of 2000" by The Wall Street Journal, was used to promote the debut of Aflac's first accident policy in Japan.

Aflac Japan sells a variety of insurance policies, including cancer, life, medical, and other life insurance products. 'Ever', a whole life medical insurance product introduced in early February 2002, had over 500,000 policies sold in one year. Policy benefits include fixed lifetime premiums, lifetime coverage, and a policy entry age from zero to 80.
